Health – Advance Care Planning w/ The Dorothy Ley Hospice

Advance care planning (ACP) is the process of thinking about what matters most to you in your life and communicating these wishes to the person(s) you want to make health care decisions for you, in the event that you cannot. In this presentation we explore some legal considerations for ACP, but mostly we encourage reflection on your hopes, wishes and preferences for your personal care; and we encourage starting the conversations with your trusted person(s) on how they can speak for you, if called upon. ACP is about reflection and then important conversations.
